Diabetes Care Process - FOR ALL DIABETES PATIENTS TO READ.

All our diabetic patients are invited to attend for a full annual review.  We invite patients through text and telephone.

All diabetic patients who have a high HbA1c (blood test checking your sugar levels) will have a results call with the diabetes nurse and will be invited in three months after a high result a repeat bloods test.  The HbA1c and cholesterol will both be checked. 

All diabetic patients who have a good HbA1c results at the annual review will be invited in for a 6 month blood test (HbA1c, cholesterol) and asked to undertake a 4 day blood pressure reading.  Once this has been done and the results are still good/stable you will not be invited in again until the next annual review. 

It is really important that you keep good control of your diabetes, cholesterol and blood pressure so please do engage with the Practice in trying to achieve this.
