Lasting Power of Attorney
Please see below what the Surgery requires to record a LPA for a patient.
If the LPA was registered after 1 January 2016 the attorney (person who has LPA arranged on behalf of the patient) should send you a 13 digit online access code for you to view the LPA summary https://www.gov.uk/view-lasting-power-of-attorney
If the LPA was registered before 1 January 2016 the person needs to show you the original paper LPA or a certified copy of it to enable you to verify the details.
Even if a patient has LPAs organised, if the patient still has competence the team will not do anything without consulting directly with the patient to seek their authorisation to discuss the record with their LPA. Officially a LPA does not come into force until the patient concerned has been assessed as not having capacity by a clinician.
